New User Onboarding Form

A minimum of one business day notice is requested for all new user onboarding requests. While we will make every effort to accommodate last-minute requests, we cannot guarantee fulfillment within this timeframe. If there are multiple office locations, use this field to note the specific location for this user.
Should an email account be setup for this user?
Should a Microsoft 365 Account be created for this user

What computer will this user use?
To find the computer name, press the Windows Key, type About and select About this PC.
Note all groups (security groups, distribution lists, teams, etc.) that this user should be added to, one group per line.
Please list each / any network drive that the user should have access to, one per line.
Please list each / any Sharepoint Site that the user should have access to, one per line. If your organization does not use SharePoint, leave this field blank.
Note all applications that the user will need to have on the computer, one per line.
